POSNA ACTIVE MEMBERSHIP

This membership is limited to those orthopaedic surgeons who reside and practice within the United States or Canada, who are certified by the American Board of Orthopaedic Surgeons or Royal College of Physicians and Surgeons of Canada or the American Osteopathic Board of Orthopedic Surgery, and who devote at least seventy-five percent (75%) of their professional time (clinical and research) to pediatric orthopaedic surgery. Active membership will be maintained by compliance with dues and the attendance of at least one Annual Meeting every four years.

• Three Sponsor Forms from Active or Senior POSNA members*

• Application fee

*Ask your sponsors to log into the application process to submit the sponsor form. Sponsors should be directed to STEP 3 on the application process webpage.

**The photo should be a head shot photograph in a digital file format, high resolution preferred, 300 dpi (minimum resolution) and minimum photo size at least 4”x6”. This will be used for the Membership Directory and New Member Board once you become a member.

Application fee: There is a $50.00 application fee, payable online using Visa or Mastercard or American Express.

Application form documents should be downloaded and saved to your computer. Then fill them out, save them, and later attach them during the on-line application process. You must use a PC computer.

If you cannot pay the application fee by credit card on-line, application fee can be sent by check in US$ and mailed to the POSNA Office at 6300 N. River Road, Ste 727, Rosemont, IL 60018-4226. Once payment is processed you will be able to log in and submit your application documents.
